Congenital Biliary Atresia - Operative results and prognostic factors -




Abstract
We evaluated prognostic factors from 34 cases of biliary atresia which had been operated on during past 13 years between Nov. 1978 and Nov 1992.
Thirty one cases were non-correctable type, and 3 cases were correctable type. Roux-en-Y portojejunostomy(Kasai operation) was performed on patients with non-correctable type and Roux-en-Y hepaticojejunostomy of correctable type. Five survival
rate
was
42%(by Kaplan-Meyer method).
We investigated followings as a important prognostic factors retrospectively: 1) the age at operation, 2) diameter of the bile duct at porta hepatis, 3) liver histology, 4) postoperative ascending cholangitis and 5) preoperative liver function
tests, 6)
TORCH study with hepatitis B virus. Results showed that the preoperative level of serum SGPT, diameter of bile duct t porta hepatis, liver histology, age at operation and postoperative ascending cholangitis were statistically significant between
survival group and mortality group (p<0.05). The age at operation is the most important progsnostic factor and early aggressive diagnosis with high suspicion should be considered in patients with jaundice before 60 days of age.
